Required Paperwork for All Summer Counselors and Camp Deans

The following forms need to be completed:

• Application
• Clearances (see below)
• Consent and Release Form
• Applicant's Certification
• Three Letters of Recommendation

EVERY FIVE YEARS clearances need to be renewed, UNLESS the following applies: If the employee/volunteer has moved to a different county or has had a break in service, clearances need to be renewed. Out of state staff DO NOT need clearances; the three references are a must!
